Understand the next step before you arrive.
Call the office to confirm participation in your specific insurance plan, new-patient availability, and any payment questions before scheduling.
Confirm your plan and availability
Insurance participation and new-patient openings can change. The practice’s published information lists its Medicare and MassHealth/Medicaid panels as full for new patients. Please call to confirm current availability.
Insurance arrangements are changing. Speak with the office about your exact plan before your visit.
Patients with insurance
Bring your insurance card to every visit so staff can verify eligibility. Copayments and prior balances are collected at the time of service.
Coverage, deductibles, and copayments vary by plan. You are responsible for understanding your plan’s benefits and any amounts it does not cover.
Billing and payment plans
Patient statements are sent monthly. If you are having financial difficulty, ask the office about a monthly payment plan.
Our billing policy requires payment within 30 days of receiving a bill. Balances unpaid in full 90 days after the original bill are turned over for collection, and the patient remains responsible for the balance.
Self-pay visits
For self-pay visits, payment for office services is expected during check-in. The office accepts cash, checks, and credit cards. Call before your visit to discuss expected costs.
Insurance referrals
Check whether your insurance requires a referral before seeing a specialist. The office requires two business days to process referral requests.
Ask the office how to send the necessary information securely.
- Patient name and date of birth
- Insurance plan and identification number
- Specialist’s name and NPI number
- Specialist’s office telephone and fax numbers
- Appointment date and number of visits requested
Let’s get to know you.
