Circle of Life is not a Direct Primary Care (DPC) practice. We continue to accept and bill insurance for covered primary care services.
The membership fee is separate. It covers enhanced services that insurance does not cover. Copays, deductibles, and the terms of your insurance plan still apply.

Individual, couple, and family memberships are available. Family plans are for people in the same household. If your household membership changes, ask the office how your rate will be adjusted.

Do I have to join to remain a patient?
No. Membership is optional. Standard insurance-covered primary care visits remain available to all patients. Enhanced membership services are reserved for members.
Does membership replace my health insurance?
No. Circle of Life is not a DPC practice. Covered primary care visits continue to be billed to your insurance. Membership covers enhanced services separately.
Are same-day appointments available without membership?
The membership program reserves same-day acute care slots for members. Non-member same-day appointments are subject to availability. Call the office to discuss your needs.
Can my whole family join?
Yes. Family memberships are available for people in the same household. The office can explain current rates and how adding family members affects your plan.
How do I enroll?
Contact the office at (339) 469-2707 or staff@circleoflifefm.com to ask about enrollment. Please keep ordinary email to administrative questions and ask the office how to share medical information securely.
How do I cancel?
You may cancel at any time by a written request to the office by email. Phone cancellations cannot be processed. You will be asked to acknowledge that you are opting out of enhanced membership services. Standard insurance-covered primary care remains available.
